Diabetes Management
Diabetes affects more than 25 million Americans. To ensure good health, diabetic patients need to appropriately manage this chronic condition. Without proper management, unmanaged diabetes exacerbates other conditions and lead additional illnesses and or death.
The Linden Center Diabetes Management Program helps patients ensure good health by bringing together an interdisciplinary team of physicians, nurses, and dieticians to comprehensively care for residents with diabetes. To manage the disease and help prevent complications, we regularly provide:

Eye exams
Our dieticians plan meals for each resident according to their needs and preferences. Residents enjoy a balanced diet that spreads carbohydrates throughout the day to prevent high blood sugar levels after meals. Nurses and dietary staff also provide instruction to help residents maintain independence and control their diabetes.
Residents are also encouraged to participate in exercise or other physical activities to improve insulin response and management.
