Admissions

 

 

The Linden Center for Nursing and Rehabilitation is a non-discriminatory facility in which no one will be denied admission due to race, color, creed, religion, country of origin, sex, age, handicap, marital status, sexual preference or source of payment as long as they meet the established and approved criteria.

 

We admit patients 7-days-a-week and accept Medicare, Medicaid, private pay, and most insurance plans.  We will work with you to explore all eligible funding options.  The Admissions Office will work with you in applying for Medicaid to supplement all primary insurance plans.

 

We encourage families and incoming residents to tour Linden Center prior to admission. We will answer all of your questions and explain the admissions process.  To set up a tour, or to request information, please contact:
