It also helps ease the future transition in the event a higher level of care is needed.

 

Hospice Care

Linden Center for Nursing and Rehabilitation’s Hospice Care program provides support to your loved one at the greatest time of their need.  We hold fast to the belief that your loved one has the right to compassionate care and that your family deserves and receives the necessary supports and accommodations. We ensure the finest quality of life for your loved one through our team-oriented approach, which is designed to meet his or her medical, emotional and spiritual needs that are based on their expressed needs and wishes. 

 

Palliative Care

Linden Center’s Palliative Care Program provides comfort and relief to a loved one suffering from severe pain or other distressing symptoms. Our program integrates all facets of care—medical, psychological and spiritual care and support—delivered by a multi-disciplinary team approach to enhance the quality of life for our patients.  In addition,our program provides support to the family of the loved one as we recognize the importance of the family members in the care process.
