Long Term Care Unit

 

The Bronx Park Rehabilitation and Nursing Center staff is dedicated to the delivery of long term care with dignity.  The facility offers the long term resident spacious dining rooms, lounges and a large activity center where they develop friendships with both staff and peers.

 

Admissions

New admissions are welcomed by staff and are given an opportunity to learn about services and life at the Bronx Park Rehabilitation and Nursing Center.

 

Clinical Care

An interdisciplinary team which includes medical, nursing, social services, rehabilitation and dietary individualize a plan of care for each resident. Nutritious meals are served with attention to individual preferences. Individual and family counseling are provided by the Social Service department.

 

Recreation

The Recreation department assists each resident to develop a schedule of activities that reflects their own interests and hobbies. The Bronx Park Rehabilitation and Nursing Center provides activities seven days a week.  Evening activities are also offered.  Sporting trips, holiday celebrations and community trips are but some of the special highlights.

 

Family Involvement

The Bronx Park Rehabilitation and Nursing Center encourages families to participate in the resident's life as much as possible.  24-hour visiting provides flexibility for family. Families are invited to attend the interdisciplinary care planning meetings. The Bronx Park Rehabilitation and Nursing Center provides a full range of services essential to the provision of a dignified lifestyle.
