How do I know if Bronx Park Rehabilitation and Nursing Center is right for me or a member of my family?
We understand that choosing a short-term or long-term care facility can be a stressful time in a family's life. That is why we will handle all aspects of the admission process for you. We invite you to call to set up a tour and speak to our experienced Admissions Director so we can answer all of your questions.
What type of insurance does Bronx Park Rehabilitation and Nursing Center accept?
We accept Medicare, Medicaid, private insurance plans, many managed care plans and private pay.
When does Bronx Park Rehabilitation and Nursing Center accept admissions?
We offer same day admissions, including weekends and evenings, 7 days a week including holidays.
Does Bronx Park Rehabilitation and Nursing Center accept people from out-of-state addresses?
Yes, we accept patients from every state. Our admissions department will coordinate with you so that all necessary forms can be completed.
Does Bronx Park Rehabilitation and Nursing Center accept individuals under the age of 65?
Yes we do. Our short-term stay / subacute unit include patients who are in their 40's - 60's. Our activities & dietary departments have special programs & dietary selections to accommodate all age groups.
Does Bronx Park Rehabilitation and Nursing Center provide hospice care and/or end of life care when a patient is terminally ill?
Yes, we provide both hospice care and palliative care to those in need. We have contacts with various hospice organizations and our interdisciplinary care plan team meets with them regularly to follow thru on health care decisions that is in the best interests of the patient and their family.
What are the steps for admission to Bronx Park Rehabilitation and Nursing Center?
Prior to admission, required forms by New York State such as the PRI (Patient Review Instrument) and SCREEN must be completed and sent to us. These forms are reviewed by the admissions team to determine eligibility.
How do I start the admission process?
We do everything we can to ease the burden for you. If you or your loved one is currently in:
A hospital - the social worker or discharge planner will send the PRI and SCREEN to us.
The community - call us and we can give you the name and phone numbers of agencies that will complete the PRI and SCREEN and they will send them directly to our admissions office.
Another nursing home - just tell the social worker to send all the necessary forms to us and we will handle the rest.
