Planning for Discharge

 

Although we want you to stay as long as necessary, you should prepare for discharge at any given time, based upon your current status, progress and insurance. 

When your final discharge date is set, the following takes place: 

1. The Therapy department will call your family members or any person you designate to learn transfer training, review exercises, etc. to help you maintain and reinforce the techniques you were taught during your stay. Typically this is scheduled approximately 72 hours-1 week prior to your discharge.

 

Order any necessary equipment (for example – walker, wheelchair, commode, etc.). Depending on your insurance, some items may not be covered but can be purchased on your own. For convenience, therapy can arrange to have equipment delivered to you prior to leaving.

 

Medicare and most insurance companies stipulate that equipment for home use cannot be ordered more than 48 hours prior to your discharge, but we will make every effort to have your equipment delivered to you here prior to discharge. 

 

2. The Social Services department will refer your information to a home health agency, such as Visiting Nurse Association, etc. If you have preference for a certain agency or have used a specific agency in the past, please mention this during one of your many discharge planning sessions.

 

3. If you decide to leave earlier than the team has recommended, we will do our best to work with you as long as the therapy and nursing staff can affirm that is a safe discharge. In order to do this, we will need a minimum of two days’ notice as we need to coor-dinate getting medications and any equipment or services you might require upon discharge.  If you are requesting a discharge that is not deemed as safe, an Against Medical Advice discharge will need to occur
