Short-Term Rehab
The Rehabilitation Team at Fairview brings hope, perseverance, resourcefulness, creativity and dedication to our residents. We implement out-of-the-box thinking to help our residents overcome obstacles and achieve success. Our rehab department provides therapy for residents who have suffered a stroke, orthopedic injury or surgery for joint replacement, traumatic brain injury or other neurological conditions. Physical, occupational and speech therapy are offered. Additionally we enjoy a doctor’s office on premises. We enable residents to take back control and aim for independence. Together we find ways to adapt function to their abilities and to help them achieve more than they thought they would.

Re-Hospitalization Prevention
Our Proactive Disease Management Education program teaches our residents about disease processes, symptom management and behavior modifications.

Full nursing assessment of the patient and the home environment.

CHF Weight Guidelines are carefully reviewed and documented with the patient and caregivers.

We focus on physical status (such as weight, edema status, oxygen use, etc.), self management behavior modification, and teaching (ie. occupational therapy, energy conservation).

Telemonitors are incorporated into our plan for High Risk Patients.

Emergency Care Plans are reviewed with the patient.

We emphasize the importance of communication between the resident and the physician, pharmacist, and caregivers.
Long-Term
Fairview Nursing Care Center offers a full range of services aimed at helping those residents with Alzheimer's and Dementia. Our program lets residents enjoy a sense of independence while still being under the constant watch and care of our skilled staff. Based on our residents' specific needs we design an individualized care plan for each resident. We try to identify things that are unique to the resident and help them decorate their rooms with things from home. Our comprehensive care programs maximizes results and gives each resident what he or she needs in terms of care.
Amenities
Our dedication goes well beyond the medical services we provide. We encourage our residents to stay active and involved in our community and offer many programs, services, and amenities to assist in this goal.
We take a tremendous amount of pride in the nutritious meals served from our kitchen. Registered dietitians review what each resident needs for necessary nutrition and works with families and residents to tailor food choices. Our main Dining Room, also known as "The Grand Jewel", greatly enhances resident dining and plays host to our monthly resident birthday parties as well our famous monthly candlelight dinners. Our recreation staff provides programs that meet the needs, capabilities and interests of our residents. It is the goal of our recreation program to keep each resident interested and their minds active.
